Consent Form

This form is required to be completed by all JST Coaching & Training participants.

JST Coaching and Training acknowledges the importance of protecting your privacy and your personal information. To that end, we request your permission to share your personal information – specifically your name, email, phone number and general location (city & state, province or region) – with the JST Team and your fellow training participants. In turn, you agree to keep any contact information you receive private and confidential.

Additionally, JST records all training classes in order to allow participants the opportunity to review a past lesson or to make up a missed lesson. Videos will be posted on YouTube via a private, unlisted link that will only be made available to course participants and the JST Team (faculty and staff). We ask that you keep this link private and confidential.
- I consent to being video recorded with my fellow classmates by JST Coaching & Training. I understand that this consent is perpetual, that I may not revoke it and that it is binding.
- I DO NOT CONSENT to being video recorded by JST Coaching & Training. Therefore, I will be responsible for turning off my camera during all training classes.
